Please submit the following:

  • Your completed information application
  • A photo of your dog
  • Completed letter from your physician stating the need for a service dog
  • Current veterinary records
  • A letter from an AKC approved Therapy Dog Trainer stating that your service dog meets the obedience and temperament requirements established by the SDRC. Click here for a complete list of AKC approved Therapy Dog Trainers
