This application is for upcoming service dogs that are still in training or not yet certified. This application is intended for:

  • Service dog trainers
  • Service dog training facilities
  • Personal trained dogs
  • Non-profits established to provide service dogs

If your dog is currently an active certified service dog, please visit our Current Service Dog Registration Page.

Please submit the following:

  • Your completed information application regarding any service dogs you have trained
  • A photo of each dog
  • Completed letter from a physician stating the need for a service dog
  • Current veterinary records

Upon obtaining the information application and required documents and a one-time payment of $85.00, an appointment will be set up for you to meet with a SDRC trainer via Skype who will ask you to take your dog through a required obedience and temperament evaluation.  Upon approval that a dog has the temperament and obedience needed to be an active service dog, the dog will be registered with the SDRC and a registration badge and certificate will be issued.
